Booking Your X-Ray:

STEP 1:

Download the X-Ray Requisition Form HERE (requires Adobe PDF Reader), Print, and Fill it Out.

STEP 2:

An X-Ray requires the Referral of a Physician. Make an appointment with your doctor and bring your form with you (or ask your doctor to print one). If upon examination your doctor confirms that you are a candidate for a scan, have your doctor fill out the Physician Information box. If you do not have a physician, you can be examined by one our certified doctors at the Urgent Care Centre.
